The New York Care Coordination Program is a not-for-profit organization governed by individuals from multi-county and multi-stakeholder groups aimed at improving outcomes for individuals with serious behavioral health conditions.
Six western and central counties came together in 2000, with support from the NYS Office of Mental Health and project management through Coordinated Care Services, Inc. They formulated a plan for system transformation, including needed structures and specific practice initiatives which became operational in 2002. A partnership was formed with Beacon Health Strategies, LLC in 2009 to bring managed care competencies to our work. In 2011, NYCCP incorporated and was awarded the contract to be the Behavioral Health Organization for the Western Region in New York State. In 2012, NYCCP was approved to provide Health Homes in 21 counties in Western New York. Learn More about the history of NYCCP
