Physical and Behavioral Health Integration Training: Providing Integrated Person Centered Care Coordination for Individuals with Complex Needs
Persons with severe mental illness are dying an average of 25 years younger than the general population as a result of co-occurring physical health conditions such as cardiovascular disease, metabolic syndrome, smoking, and diabetes. This 2-day training is specifically designed for behavioral health care coordinators (case managers) to understand their role and responsibilities related to coordinating physical and behavioral healthcare services. The training provides specific examples of how care coordinator’s practice can be modified while still meeting the requirements for Medicaid reimbursement.
