Traditionally, mental health care had a goal of "stabilizing"
individuals. Stability in this sense often meant managing
an individual's symptoms through continued medication and
involvement in the mental health system. The goal of person-centered
practices, tools, and planning is to help people with a
diagnosis of mental illness truly recover.

Family Involvement
Family involvement is important to the success of the Western
New York Care Coordination Program. If you have a family
member who is enrolled in the program and would like to
know more, please contact one of the WNYCCP Peer and Family Coordinators listed on the Staff page.
