Welcome to the Peer and Family page. If you are enrolled in the program, have a family member who is enrolled, or are simply interested in the program, we would like to hear from you.

The focus of this page is INDIVIDUAL RECOVERY:

Defined by the individual
Spelled out in an individualized service plan for a productive and safe future
Facilitated through a person-centered planning process

The goal of this page is to educate, encourage involvement, and support participation in the Western New York Care Coordination Program.

The Western New York Care Coordination Program includes a Peer and Family Advisory Group. This group is made up of two representatives from each of the six counties participating in the Program. Three representatives of the twelve member group are voting members of the Steering Committee, the governing body for the Program. The group is asked to give input from a peer and family perspective on program design and issues. The group meets the second Tuesday of each month in Batavia. The county representatives are:

The three representatives who sit on the Steering Committee are Colleen Bellezza, Lisa McChesney and Sharon Ward.

Work has begun on developing local peer and family advisory groups. These groups meet monthly to discuss county specific and program wide issues. They vary in size and inclusion and will be working on becoming more involved in the program at the county level. These groups and others will form a network of peers and family members to support the Steering Committee’s efforts to bring about change.

Family involvement is important to the success of the Western New York Care Coordination Program. If you have a family member who is enrolled in the program and would like to know more, please contact the Peer and Family Coordinator.
